SamNor: The presence of gingivitis and subgingival calculus at baseline and 6
years later was associated with the occurrence of even higher diseas
e progression during this period. The association between gingival in
flammation and subgingival calculus and the development and progressi
on of attachment loss during the study period in the generalized and
the localized EOP groups was significantly higher than the associatio
n in the incidental EOP group. In an appreciable % of the sites in al
l 3 groups, however, the presence of the 2 factors was not associated
with attachment loss during 6 years. The results suggest a significa
nt association between gingival inflammation and subgingival calculus
and the development and progression of early-onset periodontitis.
SamEng: This study was undertaken to test the hypothesis that gingival inflam
mation and dental calculus are important determinants of the developm
ent and progression of early-onset periodontitis. The study sample in
cluded 156 individuals who were 13-20 years old at baseline and who w
ere examined 2x during 6 years to assess the attachment loss, gingiva
l state and the presence of dental calculus. 33 (21), 62 (40), and
61 (39) individuals were classified as having localized, generalized
, or incidental EOP, respectively. The results showed an increase in
the % of teeth with overt gingivitis and subgingival calculus, and al
so an increase in the % of teeth showing attachment loss during the 6
-year period in all classification groups. Of teeth with 0-2 mm attac
hment loss at the beginning of the study and which developed > or = 3
mm attachment loss during the following 6 years, there were 2x as ma
ny teeth with overt gingival inflammation, and 4x more teeth with sub
gingival calculus at baseline than teeth without. Gingivitis and subg
ingival calculus when present at both examinations resulted in a stro
nger association with the development of new lesions than presence of
these variables at baseline. Teeth with gingivitis at baseline had a
significantly higher mean attachment loss during 6 years than teeth
without gingivitis (p<0.0001), and teeth with subgingival calculus at
baseline had a significantly higher mean attachment loss than teeth
         without subgingival calculus (p<0.0001).

SamEng: The study aimed to investigate caries prevalence in children using a
severity grading diagnostic system, and to evaluate the influence of
different diagnostic thresholds on the caries data. A group of 513
children, aged 5, 12 and 18 years, were examined clinically, and with
available bitewing radiographs, by four calibrated examiners. The
inter- and intra-examiner reliability, assessed by a weighted kappa,
         varied between 0.80 and 0.95. The mean dmft/DMFT values were 3.8, 5.8
and 11.0 for the three age groups respectively, and the correspond
         ing dmfs/DMFS values were 5.4, 9.9 and 22.6. The d/D-component
constituted the major part of the dmf/DMF index in all age groups,
and enamel lesions accounted for 59%, 89% and 86% ofthe d/D-component
in the three age groups respectively. It is concluded that enamel or

SamNor: When patients were grouped as smokers (8) and non-smokers (12), no si
gnificant differences were revealed for any of the response variables
. Overall, among the 20 membranes sites, one showed no change while t
he remaining 19 gained root coverage at the 6-month examination. Five
sites obtained coverage to the CEJ. Among the non-membrane sites, al
l gained root coverage at 6 months and 10 sites showed complete cover
age to the CEJ. It was concluded that the coronally positioned flap o
peration offers a predictable, simple and convenient approach as a ro
ot coverage procedure in Miller Class I and Class II recession defect
         s. Combining this technique with the placement of a biodegradable mem
brane does not seem to improve the results.
SamEng: The present study aimed to compare the clinical outcome following tre
atment of localized gingival recessions by a coronally positioned fla
p procedure alone, or combined with a biodegradable membrane. Twenty
patients with buccal bilateral Miller Class I or Class II gingival re
cessions in cuspids or premolars participated in the study. The split
mouth design, randomized selection of site treatment, and blind evalu
ation provided 20 sites in a membrane group and 20 sites in a non-mem
brane group for examination at baseline, 3 months and 6 months post-o
peratively. Clinical variables included the apical extent of the ging
ival recessions, the width of the recession defect measured at the CE
J, the width of keratinized tissue at the recession site as well as p
robing depth and attachment level. Both treatments resulted in a sign
ificant gain of root coverage, amounting to 2.3 mm in the membrane gr
oup and 2.5 mm in the non-membrane group at the 6-month evaluation (p
<0.0001). There was no significant difference between the treatments.
Similarly, a significant gain of attachment level was seen in the me
mbrane (1.3 mm; p<0.001) as well as in the non-membrane (1.5 mm; p<0.
0001) group, but without a significant difference between the groups.
The reduction of the recession width from baseline to 6 months was s
ignificantly greater for the non-membrane (2.3 mm) than for the membr
ane (1.4 mm) group (p<0.01). Probing depth changes were small and not
significant for any of the treatments.

SamEng: This study explores the influences of parents' behavior and the
parent-child relationship upon adolescents' smoking, oral hygiene
performance and intake of sugared foods. In 1993 (Time 1), a
representative sample of 709 16-yr-0ld adolescents in Hordaland
County, Norway and their parents completed questionnaires at home. A
postal follow-up 2 yr later (Time 2) provided 781 answers. The
present analyses included 597 cohort participants who responded on
both occasions, and 460 mothers and 399 fathers who replied in 1993.
Influence of the parent-child relationship was assessed in terms of
two scales: parental support and parental control of the youngsters.
Multivariate regression analyses indicated that adolescents' oral hea
lth behaviors at Time 1, were each powerful predictors of their
corresponding behaviors at Time 2. When the Time 1 parental behavior
and the Time 2 parent-child relationship scales were added, mothers'
oral hygiene performance and sugar intake accounted for a significant
amount of the variance in the corresponding behaviors of adolescents
at Time 2. Fathers' smoking and the parental control scale
contributed to the prediction of adolescents' Time 2 smoking. These
results extend previous ones obtained from cross-sectional studies
and give furth er support to the importance of including the family
in prevention campaigns aimed at adolescents.

SamEng: AB: There is a need for a descriptive epidemiology of patterns of
dental health behavior through adolescence. OBJECTIVES: The purpose
of this study to assess the tracking (degree of stability) of several
categories of self-reported dental health behavior in adolescence
over a 3-year period. METHODS: In 1992, a representative sample of
970 15-year-old adolescents of Hordaland county, Norway, completed
questionnaires under supervision at school. Postal follow-up studies
1 and 3 years later provided 709 and 781 answers, respectively. The
pres ent analyses included 581 adolescents who participated on all
three occasions and 670 adolescents who replied in 1992 and 1995.
         Paired sample t-tests, cross-tabulations and Pearson'scorrelation
coefficients were used to assess stability. For use in the
cross-tabulations all variables were transformed into binary
categories (active and inactive). RESULTS: A substantial proportion
of adolescents remained consistently in the same behavioral category
for all 3 survey years. Toothbrushing (80) and use of dental floss
         (64) were the most stable activities, while consumption of sugared
mineral water (53), intake of chocolate/sweets (52) and use of
F-rinse (54) were less stable. However, Pearson's correlations
between corresponding behaviors assessed in 1992 and 1995 were
moderate and varied from 0.59 (toothbrushing) to 0.22
(F-rinse)(P<0.001). CONCLUSION: The results do not conclusively
indicate a patte rn of tracking. The tendency is, however, clear
enough to support the assumption that dental health behavior
         continues during adolescence into adulthood.

the same proportion of deep fillings was found in both regions. A si

umber of carious lesions, but a significantly different number of fil
led surfaces indicate a generally higher caries activity in the north
rather than different treatment philosophy among the dentists in the
          different areas. ABSTRACT.
SamEng: Based on bite-wing radiographs of recruits, 20 years old and born 197
2-74, we found significantly highter DMFS scores in the middle and no
rthern than the southern and eastern parts of Norway (ORCA, 1995). Th
e purpose of this study was to further elucidate this difference by a
nalysing the distribution pattern and degree of caries within the den
tition. The bite-wing radiographs, taken in September 1994, were asse
ssed under standarized conditions by one observer. Recruits from the
south had 27% fewer filled suraces than those from the north (9.0±9.1
         , n=146, vs. 12.4±9.4, n = 240). There was, however, a significant di
fference neither in the mean number of enamel nor dentine lesions (8.
2±4.6 vs. 8.4±4.2 and 4.6±3.6 vs. 4.7±4.0). Statistical analyses indi
cated that the number of filled surfaces was indipendent of age, urba
n/rural residence and the number of teeth extracted for orthodontic r

e distribution of filled surfaces, enamel and dentine lesions within
the dentition was similar in both regions, only more surfaces were fi
         lled in the north.

SamEng: The aims of the study were to isolate and to identify at species
level the Prevotella strains in pus samples collected by needle
aspiration from 25 Romanian patients with periodontal abscesses.
Gram-stained smears and cultures on selective and nonselective media
were performed from each of the 25 pus samples. The isolates were
idintified on the basis of Gram staining, cultural characteristics
and standard biochemical reactions. The Gram-negative anaerobic
bacilli isolates were biochemically characterized and identified at
species level using the Rapid ID 32 A system (Bio Merieux, France).
Fifteen Prevotella isolates belonging to one of the following
         species: P. melaninogenica, P. denticola, P. oralis, P. loescheii
         and P. bivia were recovered.
All Prevotella isolates reached similarly in 20 tests in the Rapid
ID 32 A system. The P. melaninogenica strains showed approximately
the same biochemical profile and only two sugar fermentation tests
         were not constantly positive.
The study confirmed that Prevotella is often involved in periodontal
abscesses (> 50% of the cases) in association with other anaerobic
or/and aerobic bacteria. P. melaninogenica was the most frequently
isolated Prevotella species from the investigated cases.

behandlet i Norge. Artikkelen beskriver et kasus der en 52 år gammel

SamEng: Melanotic neuroectodermal tumour of infancy is an uncommon neoplasm
that usually occurs in children aged one year or less. Difficulty in
deciding t cellular origin of this tumour has led to numerous names,
including congenital melanocarcinoma, melanotic epithelial odontoma,
melanotic ameloblastoma, and retinal anlage tumour. Electron
microscopy and histochemical studies, however, have now established
the neural crest origin. The most frequent site of occurrence is the
maxilla followed by the skull, the brain and the mandible. The
genital or gans are the most frequent extracranial site. We present
two cases of melanotic neuroectodermal tumour of infancy arising in
         the maxilla.

es diagno Fifty-six dentists who belonged to the public dental health
service in the country of Østfold, Norway, participated in a 5 hours
calibration course. Each participant had at his/her disposal a PC wi
th specially designed software for caries calibration purpose. Two se
ts of x-ray of extracted teeth (set A and B) were used. The sets cons
isting of matched teeth according to number of sound and carious surf
aces and degree of caries (approximal, occlusal, secondary). The ober
ver’s were divided in two groups and each group had either set A or B
in the first session. After fulfillment of the first test the comput
er program gave feedback based on histological examination of the tee
th and a «golden standard» made of «experts» diagnoses of the same su
rfaces. Following a calibration session based on the computer feedbac
k and oral discussions in the groups, each observer used the other se
t in a second test. Inter-observer variation measured as mean weighte
d kappa values (compared with the golden standard) for the first test
was 0.58 and after the calibration this value increased to 0.61. The
differece was statistically significant using ANOVA with the set as
covariate (p=0.003). Conclusion: Computer assisted training can be us

SamEng: A 13-year-old girl was referred for endodontic treatment of a maxilla
ry right lateral incisor with root resorption and apical radiolucency
after orthodontic treatment. Radiographically the tooth had an invag
ination canal (Oechlers' Type III) and responded positively to pulp t
esting. The invagination canal extending to the lateral resorption wa
s opened. Vital bleeding tissue was found, and the canal was cleaned
and obturated. The pulp in the main root canal remained vital. The ra
diolucent lesion gradually decreased during the 3-year follow-up. The
bone resorption seen here, despite vital tissue in the invagination
canal, was probably due to osteoclastic activity caused by a low grad
e chronic infection, influenced by an injury-induced inflammation in
         the periodontium during orthodontic treatment.

SamNor: The median volumes of complete demineralization per treatment were: A
         , 26.3x106 µm3; B, 0.0x106 µm3; C, 4.7x106 µm3; and D, 4.4x106 µm3; w
hereby B significantly (p<0.05) lower than the other treatments, and
D was significantly (p<0.05) lower than A. With 4 subjects, 2.2 mM ch
lorhexidine and NaF+CHX were also tested. A marked bactericidal effec
t and no demineralization resulted. We show that this model is valid
to study effects of agents interfering with plaque microbiota and car
ies formation. Supp. by Colgate’s forskningsfond, Norway and Norsk De
ntal Depot’s fond, Bergen. ABSTRACT No. 1206, IADR.
SamEng: The validity of an in situ model to study dental plaque microbiota an
d enamel demineralization was tested. Acrylic appliances, attached to
teeth 36 46 and each containing two bovine enamel discs, were worn d
ay and night by 11 students during two 1-week test periods. The appli
ances were immersed 8 times daily for 10 min in a glucose/sucrose sol
ution and kept in a phosphate buffer during meals and tooth cleaning.
Twice daily each appliance was dipped for 1 min into one of the foll
         owing four solutions (A-D) using a double-blind, cross-over design: A
         : H2O (control); B: 26.3 mM NaF; C: 20 mM zinc acetate; D: NaF + zinc
. Biofilms from each disc were collected by scraping and resuspended
in acetate buffer. The microbiota were analyzed using both non-select
ive and selective culture techniques. Viability was assessed with the
live/dead stain. In addition, total cell numbers (TCN) were determin
ed with the Sytox® stain. Total CFU, total streptococci, mutans strep
tococci, black pigmented bacteria, and % live cells were within the s
ame range for each treatment. Total CFU and TCN correlated well. Eigh
t plaque samples from treatment A were further analyzed quantitativel
y for 13 taxa by immunofluorescence (IF) with monoclonal antibodies.
Great inter-individual variations in plaque composition were observed
; the sum of the 13 taxa studied constituted between 10 - 97 % (mean
         34) of TCN. The extent of demineralization was measured within a def
ined space of all posterior discs using confocal laser-scanning micro
         scopy.

SamNor: A national reporting procedure for recording of adverse reactions see
n with dental biomaterials can be implemented, and can detect changes
in profi in adverse reactions. In Norway, the relative number of ama
lgam-associated reactions has decreased since 1994. MEETING ABSTRACT.
SamEng: In 1993 the Norwegian Board of Health and the University of Bergen im
plemented a national reporting procedure concerning adverse reactions
(side effects) seen with dental biomaterials. Reporting forms are to
be returned by dentists or physicians. The reactions in question are
classified as subjective or objective ones. Furthermore, the locatio
n of the reactions is divided into local, oro-facial or general react
ions. The relevant dental procedure as well as the materials involved
should be stated by the dentist or physician. The opinion of the doc
tor and of the patient about the association between the material and
the reaction is requested. We have received a total of 674 reports.
The relative number of reports involving dental amalgam has decreased
from a peak of 89% of the reports in 1994 to 46% in 1997. Reactions
to non-associated with materials have increased from 14% in 1994 to 4
0% in 1997. Number of reports associated with materials used in fixed
prosthodontics have increased from 5% to 24%. Materials for short-te
rm and transient use involve about 30% of the reports in 1997. Concer
ning objective findings, intraoral reactions are noted in 36% of all
reports. Of those, "lichenoid" reactions constitute almost half of th
e reactions. Subjective symptoms range from local pain and discomfort
to general ill-being. Reactions associated with dental amalgam are o
f a more subjective and general nature than are reactions to other de
ntal biomaterials, and they were usually noted a long time after amal
         gam placement.

SamEng: The purpose of this study was to evaluate biomechanical, structural,
and blood now changes of femoral canal in rats 12 weeks after intrame
dullary reaming, nailing, or occlusion. In one group, reaming alone w
as perfomed. In a second group, reaming was followed by use of a tigh
t-fit steel nail. In a third group, reaming was followed by use of in
ert silicone that totally plugged the medullary cavity. A fourth grou
p served as the control. The acute mechanical and vascular effects ca
used by reaming and nailing were determined in a separate group. Ream
ing and nail insertion reduced blood flow in femoral bone to about on
e-third. Reaming reduced bending moment by approximately 40%, hereas
bending rigidity was unchanged. After 12 weeks, the cortical bone blo
od now was significantly increased in both the nailed and the silicon
e-plugged bones compared with the reamed and control groups. the bend
ing moment and energy absorption in the silicone group wer inferior t
o those of the other groups. There were no differences in either the
external or internal diameter or the medullary and net bone areas. In
the silicone group, both the number and the area of large pores (lar
ger than 10 mu m) significantly increased in comparison with the othe
r groups; hence, bone porosity was increased. The increment was confi
ned to large pores. It is concluded that medullary occlusion contribu
tes to structural and blood flow changes in bone.

tyggemuskulator og kjev Pasientene var henvist til Det odontologiske

detaljert undersøkelse tilbudt individualisert behandling. I tillegg
langtidseffekten. Konservativ behandling ga bedring i tilstanden for
over 90 % av pa sientene. Langtidsresultatene tyder på at symptomene
"kommer og går" hos mange pasienter, men de fleste lærer å leve med
symptomene når de har fått informasjon om bakgrunnen.

type 6 and type 11: both these cases demonstrated mild epithelial dys
plasia, T present study shows that a) there is a high prevalence of e
xpression of both K13 and K14 in oral lesions from Sudanese toombak d
ippers indicating dysregulation of keratinocyte maturation b) one-thi
rd of the Sudanese oral lesions expressed K19, regarded as a basal ke
ratin representing epithelial dedifferentiation, which may prove to b
e a valuable risk marker in follow-up studies c) HPV genome is found
infrequently in oral lesions from Sudanese toombak-dippers, suggestin
g that these viruses may not play a prominent role in the early stage
s of carcinogenesis in these subjects. These markers were less often
expressed in the Swedish lesions, consistent with their much lower ra
         te of malignant transformation.
SamEng: Changes in the expression of keratins (Ks), indicating disturbed tiss
ue differentiation, is one possible marker of malignant potential in
stratifie squamous epithelia. The presence of human papillomaviruses
(HPVs) in the epithelium of the uterine cervix is increasingly regard
ed as a marker of risk for cervical cancer: However, a similar role i
n oral cancer and precancer remains controversial. To address these q
uestions, potentially malignant oral mucosal lesions from Sudanese (9
          hyperplasias/40 dysplasias) and Swedish (15 hyperplasias) snuff-dipp
ers were examined by immunohistochemistry for expression of K types 1
3, 14 and 19 using monoclonal antibodies directed against each. HPV i
nfection was searched for by in situ hybridization (ISH) using the co
cktail HPV OmniProbe and the ViraType probe. For the Sudanese lesions
, moderate to intense expression of both K13 (basal, basal/intermedia
         te, basal/intermediate/superficial and intermediate/superficial cell
layers) and K14 (basal, basal/intermediate cell layers) was found in
49/49 (100). For the Swedish lesions, weak to moderate expression of
K13 (basal, basal/intermediate cell layers) was found in 12/15 (80)
and 10/15 (67), respectively. In the Sudanese lesions, expression o
f K13 showed a distinct pattern through the oral mucosa and its verru
cous projections, with an increase towards the superficial cell layer
s of dysplastic, but not hyperplastic epithelium. K19 was expressed i
n the basal cell layer in 16/49 (33) of the Sudanese lesions, while
all the Swedish lesions were negative.

nd DBH fibers were located in association with larger vessels. Round
cell-like structures within the basal part of the epithelium were CGR
P-IR. Axotomy induced a complete loss of CGRP- and SP-IR fibers in th
e anterior part of the jaws, whereas sympathectomy caused a reduction
, but not a total loss, of NPY- and DBH-IR nerves. It is concluded th
at, except for some distributional differences, the oral tissues in t
he ferret have an abundant sensory innervation similar to that found
         in other species.
SamEng: The effect of sensory and sympathetic denervation on the localization
and distribution of nerve fibers immunoreactive (IR) to calcitonin g
ene-relat peptide (CGRP), substance P (SP), neuropeptide Y (NPY), and
dopamine beta-hydroxylase (DBH) was studied in the dental pulp, peri
odontal ligament (PDL), and gingiva in ferrets. Unilateral axotomy wa
s performed by resection of the inferior alveolar nerve (IAN) 10 days
before the experiment (Group 1); sympathectomy, by unilateral remova
l of the cervical ganglion 5 days before the experiments (Group 2). I
mmunohistochemistry was performed on free-floating sections by the av
idin-biotin-peroxidase technique. A considerably higher density of se
nsory fibers IR to CGRP and SP was found in the dental pulp than in P
DL and gingiva. The majority of pulpal fibers were located in the wal
ls of blood vessels. A subodontoblastic network of fibers IR to CGRP
and SP was lacking in incisors and canines and was found only in the
coronal pulp in premolars and molars. Sympathetic fibers were sparsel
y distributed in the pulp, and they were mainly confined to large ves
sels running centrally in the root pulp as well as the larger vessels
in apical PDL and alveolar bone.

SamNor: Axotomy resulted in an early upregulation and change in the periphera
l distribution of GAP 43 in nerve profiles already 4 days post IAN ax
atomy suggesting at Schwann cell origin. Ten days post axotomy a pron
ounced upregulation of GAP 43 immunoreactivity could be demonstrated
in neurons located in the mandibular region of the trigeminal ganglio
n, compared to the contralateral uninjured side. The peripheral and g
anglionic upregulation of GAP 43 continued to persist at 21 days. A c
onmitant time-delayed shift and co-expression of NPY was demonstrated
throughout in the GAP 43-upregulated ganglion cells 10 days post axo

ilar conformity and distribution in both perinuclear regions and cell
          periphery.
SamEng: Growth associated protein 43 (GAP 43) in an acidic membrane-bound pho

otransmitters. Expression of GAP 43 has been demonstrated in the unin
jured cat dental pulp which receives its sensory nerve supply from th

SamEng: In this study, the theory of planned behavior (TPB) was applied to
predict the intention to avoid daily intake of sugared snacks and
drinks among Tanzanian adolescents. The empirical data stem from a
self-administered questionnaire surveyof secondary school students in
Kibaha district (N=532) conducted in July 1997. A total of 312 pupils
(mean age, 16.2 yr) participated. The main procedure used in the
analysis was hierarchical forced-entry multiple regression analysis.
In the final model, four variables explained 54% of the variance in
behavioral intention which was primarily governed by perceived
behavioral control and subjective norms, while attitudes and
perceived risk of having tooth decay had less impact. The appli
cability of the TPB to the study of food choices among adolescents
living in a non-occidental setting was confirmed. The results are
discussed in the light of their implication for he alth education.

SamNor: Having experienced extraction of one or more teeth during the last 5
years did not enter the equations, probably because the majority of e
xtraction involved third molars in otherwise intact dentitions (mean
=29.2, s=2.4). The resondents found it moderatly difficult to cope wi
th losing teeth and getting dentures. Gender and «belief in keeping t
eeth for life» were the only weak predictors of dental life events.
SamEng: The purpose of the present study was to (1) determine how adults rate
the need for adjustment to cope with losing teeth or getting denture
s, relati to some other life events and (2) identify predictors of fe
lt need for coping skills, in particular the effect, if any, of havin
g experienced extractions during the last 5 years. A representative,
random sample of 1.200 residents of the counties of Rogaland, Hordala
nd, Sogn and Fjordane born 1972 received a mail questionnaire plus on
e reminder requesting demographic information and 8 selected items fr
om the Social Readjustments Ratin Questinnaire (SRRQ) plus two dental
items in March 1997 (response rate 62). The items were presented as
graphic rating scales with end points "not difficult at all" (0 cm)
and "more difficult than anything" (16 cm). Mean values were calculat
ed and used to rank the life events. Respondents found «getting dentu
res» more difficult to cope with than «losing one or more teeth». (me
an = 12.4,s=3.6 and mean =10.0,s=4.0, respectively). They found it mo
re difficult to cope with the two dental life events than "trouble wi
th the family" and "being unemployed for more than 1 month" (p<0.01).
Stepwise multivariate analyses identified "belief in keeping teeth f
or life" and gender as predictors (p<0.05) of ease/difficulty in copi
ng with "getting dentures" (R2=0.046). The only predictor of "losing
one or more teeth" was gender (R2=0.017).

nxiety Scale (DAS) and Spielberger's State Anxiety Inventory (STAI-S)
to distinguish between fearful (n = 145) and regularly attending (n
= 156) Norwegian dental patients. The reliability of both instruments
was high (Cronbach's alpha indices > 0.95). With DAS, 90% of the fea
rful patients and 85% of the reference patients were correctly assign
ed to their appropriate group. Thus it may be concluded that, when us
ed on a Norwegian population, DAS is a valid instrument for distingui
shing fearful patients from those regularly attending dental treatmen
         t. The corresponding figures for STAI-S were 80% for the fearful pati
ents and 79% for the reference patients. Although not developed speci
fically for this purpose, this instrument may therefore still validly
be used to distinguish between the groups. The correlation between t

         the same phenomenon.

SamNor: The large variations in the results and the high correlations between
Hg and Ag indicated the presence of amalgam particles in the specime

n Hg concentration decreased by an order of magnitude from 11.0 ug Hg
/l to 1.1 ug Hg/l). We conclude that although Hg and Ag is highly cor
related in stimulated whole saliva from patients with dental amalgam
fillings a large part of the Hg have to be present in the form of ama
lgam particles. This has to be taken into a account when analyzing Hg
          in saliva. IADR ABSTRACT.
SamEng: The amount of Hg released into saliva from dental amalgam fillings is
debated. Hg enters saliva as vapor, ions and in particles of amalgam
. Other metals in the fillings are also released. The aim of this stu
dy was to investigate the total contents of Hg in stimulated whole sa
liva from pationts with amalgam fillings and to correlate the results
with the concentrations of Ag and Cu. Stimulated saliva (0.2 - 17.0
ml) was collected from 53 patients with alleged symptoms from dental
amalgam. The median age of the patients was 46.5 years. The samples w
ere digested in a microwave-oven and analyzed by atomic absorption sp
ectrophoto-metry for Hg, Ag and Cu. We found large variations in the
release of metals (see table, all conc. in mg/1).

kreatininjustert kvikksølvkonsentrasjon i urin. Pasientene med
amalgamfyllinger hadde signifikant høyere kvikksølvkonsentrasjon i

kreatininjustert kvikksølvkonsentrasjon i urin. Pasientene med
amalgamfyllinger hadde signifikant høyere kvikksølvkonsentrasjon i

SamEng: In previous studies we have shown that organic low-molecular compound
s are leaching from poly(methy methacrylate) denture base polymers in
vitro an vivo. The aim of the present study was to reveal the divers
ity of organic leachables from various polymer-based dental filling m
aterials. A gas-chromatography/mass-spectrometry system with an autos
ampler was used to verify the identity of the substances, which was s
eparated by the gas chromatography. The number of in vitro organic le
achables from three polymer-based dental filling materials were measu
red. No statistical significant difference were shown between the num
         ber of organic leachables from a compomer (3 M F2000), a polyglass (K
ulzer Solitaire) and a composite material (Kerr Prodigy) (p>0.05). A
tentative mass spectromtry identification have revealed following low

SamEng: According to Norwegian guidelines on infection control, debtal office
sterilizers should be routinely monitored using biological indicators
(BI). The aim of this study was to biologically monitor Norwegian
dental office sterilizers and to compare the results to those
obtained in 1986 (J Dent Res 65:611, Abstract 53) which tested steam
autoclaves (SA). Offices w ere sent: 1) a survey which included
demographic questions and inquiries concerning instrument
sterilization; 2)two sets of three BI with instructions for their use
on different daysand 3) a return envelope. Both private and public
offices participated. Completed surveys and processed BI were
returned from 163 of 300 offices with SA and from 109 of 145 offices
with dry heat ovens (DH). Testing results indicated a 6.3% overall
sterilization failure rate. Three SA (1.8)and 14 DH (12.8) did not
kill the BI. The failure rate in 1986 for SA is a lmost four times
greater than the 1996 rate. DH were over seven times more likely to
fail BI testing than were SA (x2, p<0.01). Demographic or procedural
factors such as practitioner age, gender or years in practice,
practice location, day of BI testing or location of BI within the
sterilizer could not be correlated to sterilizer success (x2,
p>0.05). Improvementin sterilizer performance may be related to a gr
eater awareness of infection control practices by dental pra
ctitioners and/or to a recent increase in the number of post graduate
cou rses in infection control offered to Norwegian d entists over the
last ten years. Study support came from the Norwegian Dental
         Association. (Abstract)

SamEng: Invasive dental treatment in pre-school children is requently connect
ed with behavior problems, and the literature is rich on how to preve
nt and de with this. One problem related to this research is the vari
ety of measures used to evaluate the treatment. The present study aim
ed at evaluating a video coding method for measuring child behaviors
and behavioral techniques used by the dental team. The subjects were
         54 3-5 year-old children in need of restorative treatment/extraction
in one mandibular quadrant. The procedure included an inferior block
anesthesia and use of rubber dam. All sessions were videotaped and th
e tapes coded for child and dentist behaviors by and independent obse
rver. The method was evaluated for test-rest reliability by re-coding
5 tapes and validity by relating the behaviors to the outcome of the
dental treatment as assessed by the operator. The reliability of the
video coding (Pearson’s correlation) varied between .784 and .999 fo

he time) than those who completed treatment. There were no significan
t differences between groups in use of behavioral techniques (64% vs
         62) and preventive holding (2.5% vs 1.3). ABSTRACT.

SamEng: This paper describes delayed development and eruption of the first
permanent molars, in most cases only in the upper jaw. The
development and eruption of the teeth are chronologically between
that of the first and second molars. The anatomy of the aberrant
teeth is closed to that of the second molars. The aberrant teeth are
often laying at a distance during dev elopment from the second
primary molars. The clinical implications of the aberrancy are few,
because the teeth will normally migrate into close contact with the
tooth mesial to them, however, often into a crossbite. Most often the
teeth are mistaken to be a first molar with some eruption disturbance
and therefore referred for surgical or orthodontic treatment. How
ever, such interception should be avoided because at least in the
present cases all teeth are observed to erupt by themselves.

SamEng: In this study the theory of planned behaviour was applied to predict
use of dental floss among adolescents. The empirical data stem from a
question survey of 15 year olds conducted in October 1992 in the cou
nty of Hordaland in Norway (N=970). Attitude, subjective norm, percei
ved behavioural contol and behavioural intention were measured at the
baseline survey, and a measure of self-reported flossing was obtaine
d in a sub-group of the original sample (n=970) four weeks later. Mul
tiple regression analysis showed that perceived behavioural control e
nhanced the prediction of intention to used dental floss over and abo
ve attitude and subjective norm. Actual use of dental floss was predi
ctable from intention and perceived control. While past behaviour pre
dicted intention beyond the components of TPB, this was not the case
for actual use of dental floss. The predicted interaction between per
ceived behavioural control and intention upon behaviour was confirmed

         BSTRACT.
SamEng: The aim was to assess the prevalence and severity of dental fluorisis
among Ugandas children, 10-14 years old, recieving drinking water co
ntaining either 0.5 or 2.5 mg.F/L, and living at different altitudes.
The children were randomly selected. They had lived permanently in t
he areas; at altitude of 900 and 2200 m (0.5 mg. F/L) or at 1750 and
2800 m above sea level (2.5 mg. F/L). Dental fluorosis was recorded u
nder field conditions on the buccal surfaces of all permanant teeth u
sing the Thylstrup and Fejerskov index. The children’s mothers were i
nterviewed about water intake and food habits during early childhood.
The prevalence of dental fluorosis increased by fluoride in the drin
king water and by altitude. In the 0.5 mg. F/L areas, 25% of the chil
dren (n=81) had dental fluorisis at 900 m. compared to 45% (n=82) at
         2200m (X² = 7.48). The corresponding values in the 2.5 mg F/L areas w
         ere 67% (n=67) at 1750 m and 84% (n=64) at 2800 m (x² = 5.25). A seve
rity of TF scores ³ 3 was recorded in 40% of the children in the high
          F-areas at 1750 m and in 50% at 2800 m. The corresponding values in
         the low F-areas were 10% at 900 m and 18% at 2200 m. The water intake
during early childhood, reported as cups per day, was not significan
tly different (X² = 2.5, df = 3) between the areas.

SamEng: Although mercury vapor is known to produce tremor and peripheral neur
opathy, neuropathological studies of the effects of the vapor are few
in number. The aim of the present study has been to evaluate the eff
ect of mercury vapor on the morphology of the dorsal root ganglion an
d the spinal nerve roots. Adult male rats were exposed to mercury vap
or for 33 days. The exposed rats developed somatic signs of intoxicat
ion and became increasingly irritable. The total numbers and volumes
of A- and B-cell perikarya in the dorsal root ganglia, the total numb
er of myelinated axons in the roots, and the cross-sectional areas of
axon and myelin in the nerve roots were estimated using unbiased ste
reological principles. The mean cross-sectional area of myelin associ
ated with nerve fibers in dorsal nerve roots of the exposed group was
significantly reduced by 20% (2P=0.014). A tendency towards a reduct
ion was seen in axon area of myelinated nerve fibers in the dorsal ne
rve roots (2P=0.087) and in the total numbers and mean volume of A-ce
ll perikarya (2P = 0.059 and 2P=0.087, respectively). No differences
between the two test groups were found for any of the parameters meas
ured in B-cells and ventral nerve roots. It is concluded that mercury
vapor, in a dose sufficient to produce intoxication, induces only mi
nor changes in dorsal root ganglion and nerve roots in rats.

SamEng: Tissue reactions to natural and synthetic braided and monofilament su
ture materials in gingiva and oral mucosa were studied. A total of 13
8 sutures made of four commonly used materials were placed in the ede
ntulous ridges and vestibular mucosa of eight beagle dogs. Biopsy spe
cimens including the suture loop and surrounding tissues were obtaine
d after 3, 7 and 14 days and processed for histologic analysis. The i
nflammatory reaction was more rapid and intense than the reaction tha
t has been reported after suture placement in skin. Bacterial invasio
n of the suture track was a common sequela regardless of the material
used, but it was particularly prominent for silk. The formation of a
perisutural epithelial sleeve was well under way at 3 days and in so
me instances included the entire suture track within 7 days. Connecti
ve tissue reactions consisted of several well-defined, concentric per
isutural zones. At 14 days, these zones were partly replaced by granu
lation tissue surrounded by a fibrous capsule. The synthetic monofila
ment suture elicited a mild inflammatory tissue response. The results
showed that sutures placed in gingiva and oral mucosa produce a prol
onged tissue response that is most likely a result of the continual i
nflux of microbial contamination along the suture channel, which may
be a lesser problem when sutures are placed in other surgical compart
ments. The results indicate that chromic gut sutures are rapidly and
unpredictably absorbed when used in an environment characaterized by
         moisture and infectious potential.

anxiety at the time when adolescents are leaving the public dental
service, and to explore the relationship at thi s stage between
dental anxiety and some possible etiologic factors. The sample
consisted of 571 18-yr-olds in high school who completed a
questionnaire and three psychometric scales, the Dental Fear Scale
(DFS), the Dental Beliefs Survey (DBS) and the Geer Fear Scale (GFS),
in the classroom. The relationship between dental anxiety (DFS and
DBS) and a total of nine possible etiologic factors was explored by
the use of stepwise regression analyses, and risk estimates (odds
ratio) were calculated in cross-tables. There were significant sex
differences for DFS and for GFS, but not for DBS. Nineteen percent of
the total sample were classified as having high dental anxiety (DFS >
59). Previous experiences of pain, phobic anxiety indexed by GFS and
pain at the last dental visit entered into the stepwise regression
models of both dental anxiety indices explaining 50% of the variance
of DFS, but only 18% of DBS. Students who reported more than one
previous experience of pain were 9.9 times more likely to report high
dental anxiety as recorded by DFS than the rest of the group. The
correspon ding risk factor for DBS was 3.4.

by the RAPID ID 32 Strep test which tested S. constellated with the
FLuo-Card Milleri test kit. Four out of the 44 isolates testing
positive with the former method tested negative with the latter one.
Rather low levels of agreement between idientifications obtained with
the two rapid test systems were obtained. In case of discrepancy
between the two methods concerning diagnosis to species level,

SamEng: The Streptococcus milleri group (SMG) is often associated with
oro-maxillo-facial (OMF) purulent infections and encompasses the
three species Streptococcus anginosus, Streptococcusc onstellatus and
Streptococcus intermedius. Identification of SMG isolates to species
level is therefore of clinical importance. Commercial rapid test kits
for such identification areavailable. The objective of the present
study was to compare a rapid method which has been used for some time
(RAPID ID 32 Strep system), witha newly introduces system (Fluo-Card
Milleri test kit). Identification by RAPID ID 32 Strep is based on
the reaction pattern of an organism in 32 biochemical reactions while
the Fluo-Milleri substrates are designed to differentiate the three
SMG species based on production of three enzymes by each. Bacterial
samples were recovered aseptically from patients with OMF infections
seem at the Emergency Clinic, the Carol Davila School of Dentistry,
Bucharest, Romania. SMG isolates were presumptive identified on the
basis of Gram-staining, minute colony size, caramel odor, and a
positiveVo ges-Proskauer reaction. Presumptively identified isolates
were further speciated with the RAPID ID 32 Strep system (bioMerieuz,
Marcy-l`Etoile, France). Fourty-four OMF isolates iden tified to
species level were retested by the FLuo-Card Milleri test kit (KEY
Scientific Products, Round Rock, Texas, USA). The level of agreement
between the identifications obtained w ith the two test systems was
         68%.

SamEng: AB: OBJECTIVES: To assess the impact of variations in radiographic
density on occlusal caries diagnosis and observers' diagnostic
strategy. METHODS: Three series of radiographs of60 extracted molars
were examined by nine dental students and caries diagnosed using a
five point confidence-rating scale. Film density in each series was
varied by varying exposure time: 1 s (high density), 0.4 s (medium
density) and 0.067 s (low density). The 'true' diagnoses were
obtained by stereomicroscopy. Diagnostic quality was measured with
ROC analysis as Az. RESULTS: The difference in Az, values between the
low and medium density series was statistically significant. The
sensitivity and the specificity values for students' diagnosesin
outer third of dentine were 85% and 50% for the high density series,
76% and 56% for the medium density series and 42% and 77% for the
light series. CONCLUSIONS: Occlusal caries was diagnosed best from
the darkest radiographs. Specificity was higher with light
radiographs but sensitivity increased with density. Therefore
underdiagnosis is more frequent with lig ht radiographs, while
overdiagnosis occurs more often with dark. The awareness of these
effects could be an important guideline in adjusting the density of
         digital images.

r left side, with the opposite side serving as the unreamed control.
The fossa piriformis (group 1) or tip of the greater trochanter (grou

mechanical testing machine until failure. In group I, all femora frac
tured through the entry portal. This happened in only 3 out of 10 in
group II, where 7 fractured through the medial part of the femoral ne
ck remote from the portal. The difference is statically significant.
Reduction in strength between the unreamed control side and the reame
         d femur was median 1486 (23) in group I and 506 N (11) in group II.
Reduction in energy absorption between the control side and reamed s
ide was median 9786 Nm (44.5) in group I and 1142 Nm (10.5) in grou
p II. The difference in energy absorption reduction between groups I
and II was statistically significant (P = 0.04). We conclude that rea
ming through an entry point (portal) in the piriformis fossa weakens
the femoral neck and creates a localizing point for fracture.

SamEng: Le Fort I osteotomies were performed in 20 patients with cleft lip an
d palate as a one-segment movement, and the fragments were fixed with
miniplat without bone grafting. Tracings of preoperative and serial
postoperative lateral cephalograms were used to determine changes in
maxillary position. The poterior nasal spine, not subjected to extens
ive changes during surgical procedures and remodeling, was found to b
e the most reliable landmark for measuring maxillary advancement and
stability. The mean maxillary advancement was 5.96 mm. Analysis did n
ot reveal significant changes in linear and angular measurement from
immediately postoperative to 6 months postoperative. A modest maxilla
ry advancement by Le Fort I osteotomy, along with alleviation of pala
tal scar tissue tension and miniplate fixation, is a stable surgical
method in patients with cleft lip and palate.

SamEng: Treatment criteria for approximal caries have changed over the last 1
5 years and many dentists seem to prefer other filling materials than
amalgam. aim of this study was to ask dentist in Norway what treatme
nt threshold they use for approximal caries, and which type of operat
ive technique and material they would prefer when they consider a les
ion distally in an upper second premolar sufficient advanced to requi
re restoration. A questionnary was sent to a random sample of 758 den
tists, and 84% responded. Of those dentists who used the outer third
of dentin as treatment criteria, 53% preferred a tunnel preparation,
while the other used either a traditional Class II preparation (23.9%
) or a saucer shaped preparation (23.1). The dentists who did operat
ive treatment of caries which was restricted to enamel radiographical
ly, used either a traditional Class II preparation (36.6) a tunnel p
reparation (34.4) or a saucer shaped preparation (28.0). The majori
ty of the traditional Class II preparations were filled with amalgam
(52.2), while composite constituted 35.0% of these fillings. Most de
ntists preferred to fill tunnel preparations with glassionomercement
or a combination of glassionomercement and composite. The majority of
the dentists who preferred saucer shaped preparations suggested fill
ing with composite (73.5), while 15.5% would fill were filled with a
combination of glassionomercement and composite and 5,8% with a resi
n modifies glassionomercement. The tunnel preparation seems to be the
most popular technique as Class II preparation. Only 20% of the dent
ists preferred amalgam for restoring a distal proximal lesion in seco
         nd upper premolar.
