SamEng: Objectives: The purpose of this paper was to study dentists' o
pinions regarding aspects of the clinical use of cast titanium restor
ations. Methods: Since 1988 more than 10000 cast titanium unit
s of crowns and fixed partial dentures have been produced by the only
dental laboratory that produces cast titanium restorations in Norway
. A questionnaire on the clinical behaviour of such restorations was
mailed to all 72 dental practitioners who were recorded in the comput
er file of this laboratory. Results and conclusion: In the opi
nion of the 64 dental practitioners who completed the questionnaire,
cast titianium is a useful supplement to conventional casting alloys,
particularly because of its favourable cost. It has a satisfactory c
linical behaviour, although there may be some problems related to tec
         hnical and aesthetic aspects.

SamEng: In vivo measurements of vertical forces and bending moments during bi
ting and chewing were carried out on 10 three-unit prostheses in the
posterior mandibles of five patients. Each patient had two prosthes,
one supported by two implants and the other supported by one implant
and one tooth. The results demonstrated no major difference in functi
onal load magnitudes related to the support type. The distribution of
load between the abutments was influenced more by the prosthesis geo
metry and implant placement than by the difference in load characteri
stics of tooth and implant. This conclusion, however, is limited to o
ne implant connected to a tooth, because multiple implants form a con
siderable stiffer unit than do teeth. An increase in vertical load re
sulting from cantilever extensions on the prostheses was documented,
both a bite fork measurements and during chewing. No substantial late
ral bending was registered, probably because the flat occlusal surfac
es and the presence of the opposing complete denture reduced lateral
         forces.

SamEng: The aim of this study was to validate the Kleinknecht's Dental Fear S
cale and the Getz's Dental Belief Survey in a Norwegian sample by 1)
testing their ability to discriminate between fearful (n=151) and reg
ular (n=160) patients, and 2) correlating them. Both instruments were
highly reliable (Cronbach's alpha >0.90). Between 81% and 95% of the
fearful and regular patients were correctly assigned to their approp
riate groups with both instruments. It may thus be concluded that bot
h instruments are valid. Also, the correlation between the instrument
s was 0.68, indicating that they to a large extent seem to measure th
e same concept. The most important predictor items for both instrumen
ts were related to avoidance of dental treatment.

SamEng: In the posterior partially edentulous jaw, implants may be used to su
pplement existing natural dentition. Frequently, the maxillary sinuse
s and the mandibular nerve preclude the fabrication of freestanding i
mplant-retained prostheses. However, if an implant and a natural abut
ment are combined, a fixed prosthesis can be fabricated, restoring th
e arch into the premolar area. The histories of three patients with a
ttachments connecting implant-retained ceramotitanium crowns with cro
wns on natural abutments are described. A design for a rigid custom-m
ade attachment for the Brånemark system, using standard components wi
         th a machine-duplication, spark-erosion technique, is suggested.

SamEng: This study presents the results from ten consecutive patients who, be
cause of insufficient bone volume for conventional implant placement
in the maxilla, were treated with an interpositional bone graft and L
e Fort I osteotomy. The endosteal implants were placed six months aft
er the osteotomy. A total of 60 screw-shaped titanium implants (Bråne
mark) were placed, of which three failed to integrate during the six-
month healing period. No further implants were lost during the follow
-up period, ranging from 15 to 39 months after placement of the impla
nts. All patients received fixed bridges and all have contiued to fun
         ction efficently.

Tittel: Re-examination of incisal tooth wear in children and adolescents.

SamEng: The material presented in this report was derived from a longitudinal
study of the development and progress of incisal tooth wear in child
ren and adolescents. The study group was established in 1991-1992 (ba
seline examination) and consisted of participants between 8 and 15 ye
ars of age. They were re-examined in 1993 (interim examination) andin
1994 (final examination). In all, 77 school children (30 females and
47 males) participated in the three examinations. The timespan betwe
en baseline and interim was on average 15 months and the period betwe
en interim and final examination averaged 16 months. The total observ
ation period was approximately 32 months. Assessments of incisal wear
was made on stone casts using the incisal wear index which was shown
to have good reproducibility. The results demonstrated that the pre v
alence and severity of incisal wear had increased significantly with
age. In general the rate of incisal wear progression was higher betwe
en baseline and interim than between interim and final examination. I
n this respect certain differences between the tooth types seemed to
exist. At baseline the relationship between age and incisal wear was
positive and statistically significant for all tooth types. At the fi
nal examination the strength of this relationship had decreased for m
axillary and mandibular central and lateral incisors. For canine teet
h of both jaws the relationship between age and incisal wear was no l
onger statistically significant at the final examination.
