SamNor: Abstract -In connection with continuing education courses in Norway
and Western Australia (WA), dealing with caries diagnosis and
treatment decisions, pre-test were conducted. Thepre-test involved
433 dentists in Norway (ND), and 62 dentists (WAD) and 108 dental
therapists (WAT) in WA, altogether 603 clinicians. Radiographs of 68
approximal surfaces were to bediagnosed and a treatment proposal was
requested foreach surface. ND showed the best overall diagnostic
quality measured by the area beneath ROC-curve (P<0.00001). The
frequency of fal se-positive diagnoses (over-registrations) on sound
surfaces varied between 7.1% (ND) and 11.9% (WAT) while the frequency
of true-positive scores for lesions in the outer half of den tine
varied from 57.4% (ND) to 48.1% (WAT). The mean numbers of
restorations suggested by the three groups of clinicians were: ND:
14.3; WAD: 13.0; and WAT: 14.5. None of the differe nces was
statistically significant. The proportion of sound surfaces proposed
         filled varied considerably between

SamNor: The quality of radiographic diagnosis of occlusal caries was examined
with special attention to the diagnosis of lesions in dentin.
Eightyfour extracted teeth, most of them with occlusal caries of
different severity and some apparently sound, were radiographed. For
validation purposes, the lesion depths were recorded during a
drilling procedure. Ten dentists in terpreted the radiographs. On
average, 12% of the sound surfaces were wrongly diagnosed
radiographically as having caries (false positives), 33% of the
enamel lesions were identifiedon radiographs, and 62% of the small
dentin lesions and 100% of the big dentin lesions were diagnosed
radiographically (true positives). There was wide variation in the
         ability to identify dentin caries correctly between the observers;
the sensitivity values from 0.45 to 0.93 (mean 0.76). The dentist
demonstrating the highest diagnostic quality (measured by the area
beneath the receiver operating characteristics curve) had only about
9% false positives (1-specificity), while the dentist who had the
lowest diagnostic quality scored caries in about 55% of the teeth in
the control group. Both of them had 96% true positives. The tendency
to make falsepositive registrations in the dentin adjacent to enamel
seems to be a major problem in radiographic diagnosis of occlusal
caries. The radiograph has significant value in the diagnosis of
occlusal dentin caries, but the great variations between observers
indica te the need for training and calibration.

SamNor: Objective: The aims of the present study were to survey the Child Beh
aviour Checklist (CBCL) scores (behavioral section) in a nonclinical
population of US urban children from low-income families and to compa
re the distribution and pattern of scores with the normative data in
the CBCL manual (1991). Method: The sample consisted of 890 low-incom
e children and a mother or female guardian selected randomly from amo
ng Seattle public school students aged 5 to 11 years. Results: In thi
s sample the total CBCL score as well as all subscale scores were sig
nificantly higher than the norms. The proportion of children who scor
ed in the clinical/borderline range was also higher than the norm. Co
nclusions: These findings support previous work showing that poverty
is a risk factor for mental distress in children. They also raise que
stions about the validity of the CBCL norms for screening or research
          purpose for low-income families.
