SamNor:  In connection with continuing education courses in Norway and Western
          Australia (WA), dealing with caries diagnosis and treatment decision
         s, pre-tests were conducted. The pre-tests involved 433 dentists in N
         orway (ND), and 62 dentists (WAD) and 108 dental therapists (WAT) alt
         ogether 603 clinicians. Radiographs of 68 appoximal surfaces were to
         be diagnosed and a treatment proposal was requested for each surface.
          ND showed the best overall diagnostic quality measured by the area b
         eneath ROC-curve (P<0.00001). The frequency of false-positive diagnos
         es (over-registrations) on sound surfaces varies between 7.1% (ND) an
         d 11.9% (WAT) while the frequency of true-positive scores for lesions
          in the outer half of dentine varied from 57.4% (ND) to 48.1% (WAT).
         The mean numbers of restorations suggested by the three groups of cli
         nicians were: ND: 14.3; WAD: 13.0; and WAT 14.5. None of the differen
         ces was statist ically significant. The proportion of sound surfaces
         proposed filled varied considerably between the groups, from 4.3% amo
         ng Norwegian dentists to 10.6% among WAT (P<0.0001). Of the l esions
         penetrating the outer half of dentine, the ND group would have restor
         ed 34.5% and the Australian clinicians somewhat fewer:

SamNor:  The quality of radiographic diagnosis of occlusal caries was examined
          with special attention to the diagnosis of lesions in dentin. Eighty
         four extracted teeth, most of them with occlusal caries of different
         severity and some apparently sound, were radiographed. For validation
          purposes, the lesion depths were recorded during a drilling procedur
         e. Ten dentists in terpreted the radiographs. On average, 12% of the
         sound surfaces were wrongly diagnosed radiographically av having cari
         es (false positives), 33% of the enamel lesions were identifiedon rad
         iographs, and 62% of the samll dentin lesions and 100% of the big den
         tin lesions were diagnosed radiographically (true positives). There w
         as wide variation in the ability to identify dentin caries correcly b
         etween the observers; the sensitivity values ranged from 0.77 to 1.0
         (mean 0.90) and the specificity values from 0.45 to 0.93 (mean 0.76).
          The dentist de monstrating the highest diagnostic quality (measured
         by the area beneath the receiver operating characteristics curve) had
          only about 9% false positives (1-specificity), while the den tist wh
         o had the lowest diagnostic quality scored caries in about 55% of the
          teeth in the control group. Both of them had 96% true positives. The
          tendency to make false-positive registrations in the dentin adjacent
          to enamel seems to be a major problem in radiographic diagnosis of o
         cclusal caries. The radiograph has significant value in the diagnosis
          of occlusal dentin caries, but the great variations between observer
         s indicate the need for training and calibration.

SamNor:  The distribution of nerve fibres immunoreactive to calcitonin generel
         ated peptide (CGRP), substance P (SP) and neuropeptide Y (NPY) was co
         mpared to the general neurochemical markers for nerves and neuroendoc
         rine cells protein gene product 9.5 (PGP 9.5) and neuronespecific eno
         lase (NSE), by use of avidin-biotin peroxidase complex method in deve
         loping dental structures in rats aged 13 to 27 days. A substantially
         greater part of the nerve fibres was immunoreactive to CGRP and SP th
         an to NPY. In the bell stage, nerve fibres immunoreactive to PGP 9.5,
          CGRP and SP were found in the dental follicle but not in the dental
         papilla and stellate reticulum. In the advanced bell stage, after ini
         tiation of dentine and enamel formation, PGP 9.5, CGRP- and SP-immuno
         reactive fibres were found in the dental papilla, while the first NPY
         -immunoreactive fibres were observed in the papilla when root formati
         on started. Concomitant with the beginning of root development, a sub
         odontoblastic nerve plexus was gradually formed and PGP 9.5-, CGRP- a
         nd SP-immunoreactive fibres were found within the dentinal t ubules.

Tittel:  Detection of mineral loss in approximal enamel by subtraction
         radiography

SamNor:  The purpose of the present study was to determine whether digital
         subtraction radiography will improve detectability of small,
         mechanically prepared defects within dental enamel. Lesions with an
         extent of 1 mm in vertical direction and representing 5% to 10%
         mineral loss in the direction of the x-ray beam were prepared in
         eight extracted molars. Radiographs ofteeth with defects were
         subtracted from radiographs taken before the lesions were prepared.
         Seven observers evaluated the images using a five-point confidence
         rating scale (receiver operating characteristic technique).
         Examination of the original radiographs showed increasing accuracy of
         radiographic interpretation with increasing mineral loss as judged
         from the ar eas beneath the receiver operating characteristic curves.
         The same observation was made using subtraction images with and
         without contrast enhancement. There was no indication that s
         ubtracted images provided better diagnostic validity than the
         original radiographs. In conclusion, subtraction images do not seem
         to improve the diagnosis of well-defined lesions with in dental
         enamel.

         ing med sprøyte fylte ikke kanalen apikalt, reamer og Lentulospiral g
         a heller ikke optimalt resultat. Spiral pluss kondensering med rotkan
         alstopper fylte 5 av 6 kanaler effektivt for begge kalsiumhydroksid-p
         reparatene. Spiral pluss kondensering med guttaperkaspiss fylte 3 av
         6 kanaler med eksperimentell kalsiumhydroksidpasta og 2 av 6 med Cala
         sept (registrert). Kombinasjonen Lentulospiral og rotkanalstopper syn
         tes å være en sikker metode for applisering av kalsiumhydroksidpasta
         i rotkanaler.

Tittel:  Variations among operators in the performance of tunnel preparations i
         n vitro

SamNor:  A study was performed to assess the variations among dentists in the
         performance of tunnel preparations for "caries" removal and cavity de
         sign, and to evaluate how these variations are affected by instructio
         n and training. Twenty-seven dentists with limited clinical experienc
         e in performing tunnel preprations participated in the study. They di
         d three partial tunnel preparations each in extracted premolars with
         standardized, artificial, approximal "carious lesions". The first pre
         paration was made without instruction, the second after instruction a
         nd under supervision, and the last without supervision. After informa
         tion/supervision, 26% did not manage to do a tunnel preparation witho
         ut residual caries, indicating that this is not an easy procedure for
          beginners. The occlusal opening was significantly larger and the wid
         th of the remaining approximal wall was significantly smaller in prep
         arations without "residual caries" than in those exhibiting "residual
          caries". Visibility appears to be improved by extending the occlusal
          opening toward the marginal ridge.

SamNor:  An occlusal restoration is usually considered necessary when a lesion
          extends into the dentin. This study aimed to assess the relationship
          between the clinical appearance of occlusal caries and the lesion de
         pth. Special attention was paid to differentiate the inner, partly de
         mineralized layer of carious dentin that should be preserved from the
          outer carious layer that should be removed before placing a restorat
         ion. Fissures in extracted premolars and molars were classified clini
         cally by the authors. The mean pairwaise interobserver agreemen t, we
         ighted Cohen's kappa was 0.73, which indicated 'substantial agreement
         '. The depth of zones revealing softness by probing and/or discolorat
         ion was assessed on ground surfaces. Ofthe carious fissures that were
          scored sound by clinical examination (n=17), 76% revealed enamel les
         ions on the ground surfaces. Of those occlusal surfaces with a discol
         oration only but no cavitation (n=57), 13 showed dentin lesions, but
         only 5 of these revealed sotfness by probing. Of those lesions with v
         ery small cavitations (n=60), 19 were confined to enamel, 41 showed d
         entin lesions and of these 32 revealed softness by probing. All but 1
          of the lesions with small cavitations (n=28) showed dentin involveme
         nt and all bu 2 of these revealed softness. If cavitation in the occl
         usal lesion was used as a criterion for identifying dentin lesions wi
         th softness, the sensitivity and specificity values were 0.92 and 0.6
         9, which means a relatively high frequency of overregistration.
